What Is Sterile Compounding
Sterile compounding is the preparation of medications that must be free from microbial contamination. This includes injectable medications, IV solutions, ophthalmic preparations, and other products that enter the body through routes that bypass natural defenses. Sterile compounding is performed in a controlled environment following strict procedures.
The Clean Room
Sterile compounding takes place in a clean room, an area specifically designed to minimize contamination. Clean rooms have controlled airflow, specialized surfaces, and regulated access. Before entering, personnel must follow gowning procedures including hand washing, donning a sterile gown, gloves, mask, and hair cover.
Primary Engineering Controls
The actual compounding work happens within a primary engineering control (PEC) such as a laminar airflow workbench (LAFW) or a compounding aseptic isolator. These devices provide a stream of filtered air that keeps the work area free from airborne contaminants. The direction of airflow and proper placement of materials within the PEC are critical for maintaining sterility.
Aseptic Technique
Aseptic technique refers to the practices used to prevent contamination during compounding. Key principles include:
- Proper hand hygiene before gowning
- Correct gowning procedures
- Never blocking first air (the clean airflow from the filter)
- Disinfecting surfaces and injection ports before use
- Minimizing movement and conversation in the clean room
PTCE Relevance
The PTCE includes questions about sterile compounding in the Patient Safety and Order Entry domains. Expect questions about the purpose of clean rooms, the role of laminar airflow, proper gowning, and general aseptic technique principles. You do not need to know every detailed procedure, but understanding the concepts and their safety rationale is important.
