Why Respiratory Drugs Matter
Respiratory medications are frequently dispensed in both retail and hospital pharmacies. The PTCE expects you to recognize common drug classes, their dosage forms, and key counseling points associated with inhaled medications.
Short-Acting Beta Agonists (SABAs)
Albuterol (ProAir, Ventolin, Proventil) is the most commonly used rescue inhaler. It provides quick relief of acute breathing difficulty and is used on an as-needed basis. Patients should keep their rescue inhaler accessible at all times.
Inhaled Corticosteroids (ICS)
Fluticasone (Flovent) and budesonide (Pulmicort) are maintenance medications used to control chronic inflammation. These are taken regularly, not just when symptoms occur. A key counseling point is that patients should rinse their mouth after using inhaled corticosteroids to prevent oral fungal infections.
Long-Acting Beta Agonists (LABAs)
Salmeterol (Serevent) and formoterol are long-acting bronchodilators used for maintenance therapy. They are typically used in combination with inhaled corticosteroids rather than alone. Common combination products include fluticasone/salmeterol (Advair).
Anticholinergic Inhalers
Tiotropium (Spiriva) and ipratropium (Atrovent) are inhaled anticholinergics commonly used for chronic obstructive pulmonary disease (COPD). Tiotropium is a long-acting maintenance medication, while ipratropium can be used for both maintenance and acute relief.
Dosage Form Considerations
Respiratory medications come in several delivery devices including metered-dose inhalers (MDIs), dry powder inhalers (DPIs), and nebulizer solutions. Knowing the difference between these devices and their proper use is relevant to the exam.
Study Approach
Group respiratory medications by their role (rescue vs maintenance), then learn examples within each group along with their brand/generic pairs and key counseling points. Understanding the distinction between rescue and maintenance inhalers is especially important for the PTCE.
