What Is a Question Stem
The question stem is everything before the answer choices. On the PTCE, stems range from a single sentence to multi-sentence scenarios. Learning to quickly identify the actual question within the stem is a key test-taking skill.
Long Stems Often Test Simple Concepts
A scenario describing a patient's medications, allergies, and insurance status might end with a straightforward question like "what is the day supply." All that background information may be context, and the actual question is a simple calculation. Do not let the length of the stem intimidate you.
Finding the Question
Read the last sentence of the stem first. This is usually where the actual question lives. Then read the full stem with that question in mind, pulling out only the information you need to answer it. This approach prevents you from getting lost in irrelevant details.
Key Words to Identify
- "BEST" - you may need to choose among multiple correct-sounding options
- "FIRST" - asks for the priority action in a sequence
- "NOT" or "EXCEPT" - reverses the logic, asking for the incorrect option
- "MOST LIKELY" - asks for the most probable answer, not the only possible one
- "SHOULD" - asks for the appropriate or recommended action
Practice Strategy
During practice exams, highlight or underline the key question in each stem before looking at answer choices. This forces you to identify what is being asked and prevents the common mistake of answering a different question than the one posed. Over time, this analysis becomes automatic and helps you work through questions more efficiently.
